Description

Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems. Participates in patient care activities related to the patient's psychosocial well being and discharge planning needs.

  • Coordinates with patient/family to assess needs and develop an individualized plan in coordination with the healthcare team. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. Arranges for needs as appropriate.
  • Performs Medicaid screenings as indicated for NHP.
  • Completes assessments (including but not limited to psychosocial, risk, and substance abuse), referrals, and follows-up regarding Adult Protective Services and Child Protective Services, as appropriate.

Qualifications

Minimum Education
Required: Bachelor's degree BSW in Social Work

Preferred: Master's degree MSW in Social Work

Minimum Work Experience
Required: Experience in health - related field or setting.


Preferred: Experience in acute hospital social work and discharge planning.
